Anyone have a good Assessment/Plan or HPI template that they utilize with midlevels to make it easy to review what the presenting complaint was and what work up/treatment has been rendered by midlevels at each visit before patient came back to see me?
Wanted to streamline the note so I dont have to look through each of the previous notes to see what has been discussed/occurred.
