Pain Management: Canada versus U.S.

Does anybody know what are the differences in practice between Canada and the U.S. Do Pain physicians implant SCS and inthratecal pumps in Canada or is that the domain of Neurosurgeons? What is the reimbursment like? Which specialist pursuit pain as a subspecialty (Anesthesia, PM&R, Neuro...)?
