Washington state requires all nurses and physicians working in level 1 (only one in the state) and level 2 (there are many) trauma centers who work in the ER, OR or ICU to have taken PALS or ATLS. It doesn't have to be current. Only applies to staff, not residents. I have met staff who have taken one of the classes, but lost their old card, and ended up having to retake it.
I have no idea if other states have similar requirements. If you take PALS or ATLS and let it lapse, hang on to your card. I'm sure a few other states or hospitals may have similar requirements.
