Do residencies know that these courses were simply pass/fail? For instance, our school has us take several two week required courses (radiology, EM, outpt. surgery) that are strictly pass/fail. Does the school send a notice on the transcript or is there something in ERAS that you have to do?

I am sure every school does this differently, but on my transcript, clerkships that are pass/fail are clearly marked as such. If you you are worried about it, then get a copy of your official transcript and see it for yourself. There is no section in ERAS where they ask you for your grades. I wouldn't worry about this.

Yep, it's usually written on the Dean's letter that certain courses are P/F only.
