Okay, silly question, but applications are driving me crazy. Has anyone submitted their dental transcript to PASS without the "Dental School Transcript Request Form"? Can I just throw in an official copy without that form in it? This would save me a trip to the registrar. Thanks.

Yeah, I just sent them an official sealed copy w/out the request form, no need for it if you have the transcript already.
