Medstudents and Residents - Does anybody have a good patient data sheet that they use on Wards available as an original file? I've got 2 or 3 that I like, but all hand-me downs from other med students, and my 3rd or 4th generation photocopies are starting to look crappy. I figured I would ask here first before wasting a few precious hours on Word or Publisher and making my own document.
I'm looking for something with a basic H&P on the front, CC/HPI/ROS/PE/Labs and then daily progress updates on the back, SOAP+Labs format. Any help, or am I all on my own? Thanks so much.
Heck, if any of the highschool and premed students who lurk in the Residencies forum have a good data sheet "Just to get ahead when they get into medical school" I'll take that too!
