PCO Domicile Certification Form...did anybody else receive this in the mail?
What would you attach for number 24? It says "please attach a statement which illustrates activities which document your intent to remain in Pennsylvania for the forseeable future".
Also it says on the bottom "Notary", so are we suppose to have someone do the notary for us and witness our signing? Hmmm...any thoughts on this may help! Thanks!
