Does it make sense to assign a LOR from your away rotations program director/department chairman to their program when applying to that very same program? Or is it better to use all letters from your home program?

Use the best letters that you have. The away rotation probably knows you already.

Yes. It would look very bad otherwise on many levels. For one, the person who took time to write you a letter will wonder why you did not include their letter. Ego is a fragile thing in derm, learn to pander to it.

There is nothing that makes a person fight for you more in closed conferences than to have that person take pride in supporting you in the first place.
