No, O2 is not a cofactor or substrate in PDH.
No. The CO2 that's produced is because Pyruvate undergoes a decarboxylation step to produce a product (which later combines with Vitamin A to produce Acetyl-CoA). Two additional decarboxylation steps occur in Kreb's cycle. I think you mean Coenzyme A, not Vitamin A. The cofactors for PDH are B1, B2, B3, B5, and Lipoic acid.
B1 = thiamine pyrophosphate
B2 = FAD
B3 = NAD
B5 = Pantothenic Acid -> CoA
