I have a question please:
How are supposed to submit the Pediatric Cardiology Fellowship Evaluation Forum?

Should we send it to the letter writer then he/she will upload it to ERAS?

Yes, with the original letter in one PDF file.

You mean the original LoR author/writer has to create a pdf containing both? and then both will be waived?

Yes, the writer has to write an original letter (like any other LOR), then fill the "Pediatric Cardiology Fellowship Evaluation Form", and attach it to the original letter as one PDF file, then upload the PDF file to ERAS
