BMJ student archive...

(1) The chest x ray film shows a normal sized heart with upturned apex suggestive of right ventricular hypertrophy. The absence of the main pulmonary artery segment gives the cardiac shadow a characteristic appearance often termed as "coeur en sabot" or boot shaped heart.

(2) The most likely diagnosis is tetralogy of Fallot

From TCH's "PLAIN RADIOGRAPHIC DIAGNOSIS OF CONGENITAL HEART DISEASE" (http://www.bcm.edu/radiology/cases/pediatric/start.htm):

"Boot shaped": Tet (http://www.bcm.edu/radiology/cases/pediatric/text/3a1A.htm)

"Egg on a string": D-TGA (http://www.bcm.edu/radiology/cases/pediatric/text/2g1A.htm)

"Snowman ([sometimes] in a snowstorm)": TAPVR (http://www.bcm.edu/radiology/cases/pediatric/text/2e-desc.htm)

"Box shaped": Ebstein's (from a different, but also cool article http://www.scribd.com/doc/4801246/-Cardiology-Radiographics-)
