Peds Anesthesia Case Question

Hey all,

I'm not going into anesthesia, and had some questions about a 'typical' peds case scenario I was given. I am having a hard time finding helpful information about it so if anyone could provide wisdom/point me in the direction of any good resources that would be great.

Case: 5 y/o male going to have T & A under general anesthesia. Weighs 38 kg, long hx of snoring/apnea per parents.

Q: What preop info is needed? Any specific things to keep in mind? What would be the best route of preop, intraop, postop management?

First of all this is a big kid for his age. If we are talking about the 50th percentile for his age, ...he should be only about 19-20 kgs.

Preop: Well if he his healthy with no problems during birth,etc then no labs are technically required. The only thing to watch out for is if he's had an upper resp infection that is purulent,etc in the past 2 weeks. If so he is high risk for larynogspasm,etc post op.

Intraop: Mask induction, start an IV, DL and put a endotrach tube. Wake the kid up when he's wide awake. This kid has sleep apnea, so make sure he is 'awake' not just waving his hands in the air. Look for good grimace and purposeful movement. If using a cuff or uncuffed tube, mk sure there is a leak around the cuff at <20 cmH20

Post op: Concerns include croup and hematoma.
