I'm not applying for it, but I looked into it. Are you confused about the app process?
Neuro has a robust "fast-track" like option (most of the programs), where you apply to advanced positions and prelim positions at some places, and some places automatically match you to their own prelim, so you know you have the whole 5 years where you match.
Also, some people finish a full 3-yr residency in Peds and then do 3 years of Neuro (similar to any other fellowship except A&I which is 2 years, and sports med, etc.)
