So I just returned from a Peer Review mtg were we asked a new surgeon as a guest to appear before us and discuss a case. For those of you that are not familiar with PR ( I wasn't when I left residency) it is not exactly M&M. It is a panel of physicians from different specialties on staff at the hospital who review cases that trigger a review. Most cases are non issue type cases where no fault is found. On occasion when a case needs to be explained in more detail than the panel can do or when there was some severe consequences we will ask the responsible physician (or midlevel) to appear before us. I won't discuss the case but what I want to do is explain what may get you through one of these situations looking like a stellar physician even in the face of grievous consequences or at least minimizing the insult.

This physician, a recent grad, came to the committee to discuss a case that went wrong. The tone is usually very cordial as most who appear recognize why they are there and have already come to terms with the chain of events and their role in these events. But in this case, this was a surgeon with either a chip on their shoulder or a lack ability to admit to anything. For an hour and one half this person explained the events without ever admitting fault. We tried to lead the discussion and offered openings for reflection and explanation of what events that may have lead to the change of events. Not once did the person admit fault or responsibility. At the end this person even tried to blame others, deplorable.

So if you ever find yourself in front of one of these panels be prepared. Know your case. Know what went wrong and why. And admit fault. Be humble and don't even think about blaming others. Trust me, the case will have been analyzed to the hilt and you are there for a reason. If anyone else has any responsibility, they will be sitting next to you. Take it as constructive criticism. If you do these things you will walk out of that room looking like a rockstar. Now if you keep getting invited back your rockstar status will be in jeopardy.
