Any suggestions for a good peripheral joint injection technique book?

Ideally, I would like to find one w/ pictures, various approaches, some text on common pitfalls, needle and steroid/anesthetic suggestions, etc.

Image Guided Musculoskeletal Intervention (Fenton and Czervionke) is great- as is their spine intervention book.
