I had a quick question as I am finishing up my personal statement. I plan to discuss a patient encounter I observed while working in the emergency room. Is it acceptable to use a first name of a patient when discussing the encounter?

No. Change the name or refer to them as "the patient," and be careful about other PHI.

So maybe I should have been more clear. I meant that I had picked a random name but how do I make sure that readers know that it is not the real name? I thought about using quotes but that seemed a little weird especially in the first line or two of the essay?

Use quotes. I see it all the time in personal statements, it is understood what you are trying to do.

Another alternative would be to simply use made up initials. Gets the point across without having to use quotes.
