Should the personal statement be single or double spaced? The limit for SUNY's app is two pages. How long were yours?

What points should I focus on: academic ability, optometry experience, experience with people (non-optometry setting)? Little help.

Is it alright to add a little humor or do I need to make it all serious?

Let your personality shine through. This is your only chance to give your dry application some jazz. A little humor is fine, but don't make it a stand up skit. You should probably mention everything you stated, but focus on what ever you can make the most interesting.
