Is it wise to say in the PS that I'm more interested in primary care versus being a medical scientist? Maybe something along the lines of... although i had extensive research experience... I find working directly with patients more fulfilling for me?

Would this have a dramatic effect in getting an acceptance into "research giants" school (i.e. top20) versus other ones?

Just asking because I'm not sure if I should include this in the PS and how adcoms would look at it from their perspectives. Any advice welcome. Thanks.

interesting - it didn't occur to me until now, but i guess the same PS has to go to all your schools, doesn't it?

i obviously have no idea (way off applying) but if i were you i'd be honest about it, because primary care people are a need right now, aren't they? and if you got rejected from a research heavy place because you don't want to do research, then it kinda seems like that might be for the best - they want researchers, after all, and you'd rather interact with patients than write IRBs and try to publish all the time...
