sorry if I sound duh but I don't understand why some residency program don't start pg-1 but start at pg-2.

Because they require an intern year before you start.

Not sure what you are asking. But if you have already done a PGY-1 in the US you are eligible for a PGY-2 position. Whether or not you get it is an entirely different story, so depending on your field of choice and the competitiveness of your application, you may wish to also apply to PGY-1 positions to increase your chances of matching.

This has nothing to do with being an IMG as the rules are the same for everyone on this issue.
