PGY-1 specialty options? is ER possible?

There isn't really a rule that would prevent it. You just have to be doing patient care. The hardest part would likely be finding a program that would take someone for one year only.

I was wondering what fields we can complete PGY-1 in. I was interested in doing an ER pgy1. Has anyone done this? Is it even possible?

Even though you would be given one year contracts as an E.M. resident, you would be expected to be willing to complete a residency in E.M. when you applied. Resident working schedules presume a minimum number of residents of each year, and your not being willing to stay beyond one year might be a non-starter in the eyes of an E.M. program director. Most programs don't usually cut residents or have non-categorical positions.

Is there indecision about ophthalmology on your part? Are you looking at E.M. as possibly an alternative? Have you applied to the ophthalmology match? If you applied, were you matched?

Medicine and TY are the usual picks, pre-ophth, surgery for the masochists, and peds (although not many).

Nope, no indecision! I am interested in international medicine and thought it would be a good idea to get some acute care experience and pgy1 seemed to be the best time to do that, especially since I've been hearing that most of what you learn in intern year isn't applicable to ophtho, sans getting experience in management of common chronic diseases that show up in the eye ultimately.
 
EM residencies are either 3 or 4 years, and of the 4 year programs, some are PGY1-4 categorical programs while others are PGY2-4 advanced with a prelim or transitional year. As mentioned, there are no 'prelim' EM years that don't expect you to go all the way through to completion of an EM residency.

Assuming you are competitive for ophtho, you could easily find a TY and stack your elective months toward EM. Might work out better for you.
