Hi everyone, I need some advice and help. I recently just finished my PGY-2 year in categorical IM. During the year i wanted to switch out of my program to either a better IM program . Unfortunately that didn't work out - due to paperwork delay on my program's part and at the end just not matching. Therefore I am not in the program anymore and left on good terms. What should or could I do? Could I apply for PGY-1 IM programs again, or does my gov't funding not allow me to do that? Do I just hope to find a PGY-2 position that opens up during the course of the year or just wait until next year? Should I change fields that require 1 year of IM? If confused and worried. Keep in mind I'm a carribean grad, 1st time pass Steps...Any advice?
 
I'm not 100% certain I understand your problem. From the above, I assume you completed two years of training in a program, no problems, and then decided to quit in the hopes that you'd find a better PGY-3 spot. Then you didn't, and so now you have only 2 years of training and no program.

If that's the case:

1. This was not a great move on your part. Few programs are interested in taking a PGY-3, simply because most residents who leave after their PGY-2 have problems. Still, it's possible -- people leave to join a research track, or simply leave a program.

2. You could try to get a new PGY-2 spot. Theoretically, you shouldn't be able to do this -- the ABIM frowns upon having someone repeat a year who doesn't need to. Still, "frowns upon" and "not allowed" are two different things. Would be a bummer to have to do your PGY-2 again, but if you got into a better program then perhaps it is worth it.

3. There is no central list of open PGY-2/3 spots. You'll need to simply contact programs and see what's open.

4. If you want a field other than IM, you'll equally have to look around to find an open spot.

5. Gov't funding may be a problem. Since you started training in IM, you have three years of funding, two of which have been used. You have one left. However, when you use up that last year, there is still some funding -- only the DME is decreased by 50%, the IME is still 100%. Therefore, many programs will not care, if they want you.
 
I'm sorry if I was unclear. I finished only my 1st year of Categorical IM. I have never done a PGY-2 year. Also, Yes I am in good terms with my previous residency( no problems). I left on good terms and left it as a leave of absence. My question is- can I apply through eras as a pgy-1 again in IM( is it worth it)? Or should I apply to other fields through ERAS , while contacting PD through e-mail for PGY-2 spots? I just want to make the best game plan considering I don't a have a great deck of cards. The finished pgy-1 year in IM can be used for many 2-4 categoricals also, but I would like to stay in IM. Then again I can't be too choosy?

Or you can call around. I bet most places don't advertise unless they're having difficulty covering their services without that resident.
 
Much depends on information you haven't mentioned yet:

1. Did you simply finish your PGY-1 (with a satisfactory performance) and not sign a PGY-2 contract? In that case, there is no leave of absence -- you simply left at the end of your contract.

Or, did you start your PGY-2 and run into problems, take a Leave of Absence, and then somehow lose your spot (or simply not finish your PGY-2).

Or, did your PGY-1 get extended due to some problem, you finished it, and then didn;t start a PGY-2.

2. IMG or USMG or DO?

3. Some sense of your USMLE's.
