Hello everyone,

I will be completing my commitment next summer and I was wondering how to apply to a PGY-2 position in a civilian program. My PGY-1 was in medicine and interested in to complete in IM or FM. Should I apply through ERAS as PGY-1 or contact the PDs individually to check if they have PGY-1 position? How difficult is to get in to PGY-2 position in IM or FM after separating the military and what is the best strategy.

When is your commitment up?
Are you planning on getting out of the military to go to residency, or stay in while getting deferred or full-time outservice?
What branch are you in?
Are you MD or DO?
Do you have any red flags (gaps in training, failed steps, dismissals)?

Sorry, I am a FS and my 3yr GMO tour end this summer but I didn't apply for military residency. I just want to get out next summer when my commitment is up. I have also edited my original question. Thank you

Check out this post I made directed toward PM&R applicants. The truth is that there is lots of information that you can use for IM/FP, and it gives you resources to check out. I also included a section for military applicants which is very relevant to your situation.

Specific to your situation...

You need become educated on the civilian match process. That starts by going to Freida Online and figure out if there are any advanced programs. You MUST know the difference between advanced, categorical, and physician spots. You must also know the timeline to apply to each. For instance, I matched to a PM&R advanced program in the 2015 match and will be starting this summer. Ideally, you would match to an advanced program. That way you would start as a PGY-2 and you will match a year in advance so that you don't have to submit your separation paperwork before finding out the match results. As it stands, if you plan on getting out summer 2017...they recommend you submit your separation paperwork summer 2016. If you want to start residency next summer, it will be with a categorical or physician spot. That sucks...for the following reasons. If you match categorical...you will have to repeat PGY-1. If you match physician only...great...you start PGY-2. The problem is that there are a significantly reduced number of available physician spots. If you don't match to either...you will be your way out of the military and you will be heading to unemployment. You could match advanced, but the problem is that you will have a gap year if you have already submitted your separation paperwork. You could potentially get your separation pulled back, but it is no guarantee...and it's not easy to do from what I understand.

You should have applied to advanced IM and FP programs THIS cycle with anticipation of getting out in Summer 2017. Oh well too late. If I were in your shoes now....I would try to figure out if FP and IM spots exist (I'm pretty sure they do). Then I would extend a year and apply advanced this coming up year to advanced programs. That way if you don't match, you aren't homeless...and you can apply military where you are essentially guaranteed an IM/FP spot.

I don't know that you can apply one year in advance for a pgy-2 spot on ERAS. Do they normally list pgy-2+ spots one year in advance like that? In my understanding, you can apply only for the following year. I wish I knew this before. In my case, I match to a pgy-1 position for that year, and I repeated my intern year again..wasted one year for nothing. Thank you for the info.

His contract is up 2017...so he could try to scramble into an advanced spot or go outside of the match this cycle. I personally wouldn't go that route because I would want the opportunity to land the best training program possible. But it is an option for the OP. Calling random programs asking for positions outside the match could be incredibly taxing...and his resume, LORs, and PS have to be ready. He's running out of time to employ this strategy.

Again...you absolutely have to understand the program types (advanced, categorical, transitional, and physician), and the timelines for each.

Transitional: PGY-1 only, start summer after match

Categorical: PGY-1 through the end of training, start summer after match

Advanced: PGY-2 through the end of training, starts two summers after match (because they assume you will be doing a transitional PGY-1)

Physician: PGY-2 through the end of training, starts summer after match (assumes that you have already completed a PGY-1)

For military applicants heading civilian...the safest is applying to advanced programs two years before your separation date. Most specialties have tons of quality advanced programs, you won't have to repeat an internship (assuming you have a qualifying PGY-1), and if you don't match...you still have your military job. I am doing it this way, and I know tons of others. It's a very smooth transition with low risk.

Thank you. If I didn't get any luck, I may stay one more year and apply in advance. Also, as rejoin5 asked above--if you match to that pgy-2 spot now, do they give you any guarantee to keep the spot for you? Like in your case, what did you do after you match? Thanks
