What is the situation for those who apply for PGY-2 positions?

Does this mean they did a year in research or as an intern? Or does it mean they couldn't match?

I noticed there were 22,000 or so PGY-1's but only 3,000 PGY-2 spots. Please help my ignorance yo!

You can only go into various specialties - derm, radiology, urology, rad onc, PM&R, anesthesia, etc - as a PGY-2. You have to have done a preliminary or transitional year prior to matching into those fields. You often apply for both spots simultaneously.

What's PGY?

Post graduate year. It refers to where you are in terms of residency training. Intern year is PGY-1, 2nd year of residency is PGY-2, etc.
