PGY- 2 Positions

What is the situation for those who apply for PGY-2 positions?

Does this mean they did a year in research or as an intern? Or does it mean they couldn't match?

I noticed there were 22,000 or so PGY-1's but only 3,000 PGY-2 spots. Please help my ignorance yo!

You can only go into various specialties - derm, radiology, urology, rad onc, PM&R, anesthesia, etc - as a PGY-2. You have to have done a preliminary or transitional year prior to matching into those fields. You often apply for both spots simultaneously.
