Hi guys,

I just started 4th year, and have made a last-minute decision to switch into Neurology. One question: Are there any residency programs that begin in PGY1, or do all programs require that you do a year of Internal Medicine first?

There is an ACGME requirement to do a prelim/transitional year with 8 months of medicine prior to starting your three years of neurology. Some programs are categorical, meaning that this medicine year is included in your "4-year" neurology program.
 
If you interview at stand alone TY's or prelim spots, just ask if they have had many neurology prelims in the past, and if they have had some they will know the ACGME requirements and won't let you schedule yourself out of your PGY-2 spot by doing something like taking 3 months of neurology elective.
