Is phone/skype interview common? I do prefer it due to logistics issue.
Is it usually a preliminary interview before an onsite interview invite? Or, can a phone interview lead to a contract?
 
This is very common. Usually an onsite interview will follow -- especially because you should probably see the place before committing to it. But they could offer you a contract without an onsite interview, if they so choose.

I've worked at and toured a lot of hospitals, and honestly the range of difference in appearance isn't that vast. If your focus is a particular specialty or geography, a tour of the wards isn't going to sway you one way or the other. Meeting more people could be of value, though.
 
I know that it sucks to spend the money...but the in-person interview is important for the interviewee. You really need to meet the residents and see what they think about the program and how you feel you would fit in the program. I am all for Skype interviews (if the program allows...which is rare) if you have a program very low on your list and you just want to ensure match. But if you are sincerely interested in the program...check it out in person.
 
How common is it for a program to rank an applicant solely based off a phone interview? I had a phone interview with McLaren FM a few months ago but got no in person interview. I decided to rank them "just in case" but had zero expectations of matching there until reading this.

You probably don't want to list a specific program unless you want them to identify you ...
