1.) I self-studied and passed the PTCE back in April. How would I fill out the Affidavit of Completed Coursework on page 4, and do I simply write "N/A" for all of the blank entries?
2.) For the copy of the PTCB certificate, could I just print out the one available online and send it as it is, in black and white?
