What did you guys wear for your 2x2 photo? I was planning on tie and button shirt but also thinking just wearing my scrubs lol.

Don't over-complicate this. It's a 2x2 photo. Take from chest up, smile, and don't look like a slob.

Yes, then or bring with you to the interview or email, should say in the secondary/supplemental application instructions if necessary.
