Not sure why you are looking for a physical diagnosis text for step 1, but first aid for step 1 is a pretty damn good book to have around during your first and second years. Also, Goljan audio is useful to listen to with systems. Search around and you will see a consensus that this is sufficient for first year, if in fact you are starting your first year. Good luck.
