I'm having a great deal of trouble acquiring a physician LOR. I have 2 science professor LORs, 1 non-science LOR, and 1 volunteer coordinator LOR. I've scribed for my dad for 1000hrs, so I could I ask him for a LOR?

Absolutely not. Use your dad's connections to set up shadowing with another doctor and ask them to write a LOR at the end.

I live in a different state. I'm emailing and calling doctors' offices atm.

Have you asked your dad if he has anyone there anyway? Beyond that, I think you're on the right track. Just an anecdote, but it took me a year to find someone to let me in their office. Keep trying and use anyone you know who might have a contact for you.
