I sometimes have problems picking out the right size SS crown. In the clinic I work in the old timers still like to do MOD alloys on Pediatric Patients and avoid SS crowns as much as possible, so I am kind of on my own when I do these procedures.
Is blanching of the tissue normal when you cement a SS crown? Is D4 the size usually used for primary 1st molars?
 
Why do an MOD on a primary tooth when a SSC is a much more predictable restoration?
 
To start its less expensive.

Why not ditch the ssc and use Fluoride releasing Fuji II to fill the MOD as opposed to amalgam.
 
Yeah start out w/ a 4...a D4 or E4 since they are the most average sizes. Work your way up from there. You can try to trim them and recrimp if you are getting a lot of blanching on the tissues but it should be fine.

Another trick to help. Sometimes with lower Ds they can look very square shaped, especially after you prep for the SSC. A good tip is that in those cases try an upper D from the opposite arch, it usually works like a charm if the tooth is pretty "square." For example if you are working on S (lower right), you can try to fit an upper left crown for I onto the S and it will work well.
