Some plastics guys work in Beverly Hills & make $2 mill & others work in the burbs & make $300K. What determines where one ends up?

Is it where you do your fellowship do you think? how good of a surgeon you are? contacts? luck?
I'm sure there are some damn good surgeons working in the burbs
