if the permanent mandibular first molar is extracted before the permanent mandibular second molar, then is it preferable to let the second mandibular molar drift mesially naturally or we just keep the mandibular second molar upright by tipping it distally and put a pontic in place of mandibular first molar??the decks say in one place first is true, and in the next deck say the second option is true.please clarify????

i should think that this Q is best answered knowing the age of the patient and the condition of the other teeth.
firstly, if the patient is fairly young and the chances of the 2nd molar drifting mesially to close the space is viable as the bone is "soft", then, the 2nd molar should be allowed to drift mesially and come into occlusion.
however, if the age of the patient does not permit easy movement of the 2nd molar into occlusion, one should place a pontic in the space created.
i am not sure if this is correct, but it is efinitely something i would consider.

correct me if i am wrong anyone...

thanks for a quick reply, we are talking abt a 9 yr old, and in the decks at one place they say move it mesially, other place they say put a pontic for same age.............so i am confused?personally i tihnk it should be allowed to move mesially......

I feel if you can find the bud of 3rd molar for the paranomic Xray, move the 2nd to mesial is the best choice. Anyway, it depends on other situations.
 
let the tooth drift ,close the space ,but dont let the tooth tip,which can be corrected orthodontically

In my opinion you can never allow a tooth to "drift"and hope that there will not be any tipping,especially a tooth like second molar you have to orthodontically guide the tooth or to upright the tooth and go for a bridge or an implant.

Since the child is 9 yr old, extraction of the first molar will cause the drift of the second molar (until it contacts with the roots of the second premolar) and not tipping. No orthodontic treatment is necessary to prevent it from tipping or to upright it.
I may (or may not) be wrong and appreciate correction.
