One of the big issues is the difference in number of years of residency training - 4 in the US, 5 in Canada. You must either go back to a Canadian PM&R residency program for PGY-5 or complete an ACGME accredited fellowship to ensure you've completed 5 years of graduate medical education.
I'm sure there are a few more issues; however, this seems the most time intensive.
