You must do a prelim year before starting your PM&R residency (for a total of 4 years). The following specialties also do a prelim/transitional year: anesthesiology, radiology, radiation oncology, neurology, dermatology, urology and ophthalmology.

Some programs offer the option of doing all 4 years with them, or you find an intern year on your own, then do the other 3 as an "advanced" option. The rest only offer the 3 year advanced residency.

and a couple (like Mayo) offer both categorical and advanced spots.

2 categorical spots = PGY-1 at Mayo Prelim + Mayo PGY2-4 PM&R
6 advanced spots = find your own intern year then Mayo PGY2-4 PM&R

For those "advanced" programs that are 3 years (as opposed to the 4-year "categorical" type), does the MS-4 match a whole year in advance or do they match PGY-1 at St. Joe's for an internship year and then submit a ROL via ERAS during that intern year? Or, do they match both in the same application and year.

Where does the DO traditional internship year fit in to all this? Would a 3-year program accept a DO who matched in trad. intern that same year or does participation in the osteopathic match preclude one from participating in the allopathic match that same year? Also, would a 4-year program accept a DO who has completed the trad. internship year and grant PGY-2 status? Would a 4-year program accept a DO right out of MS-4 without having completed the trad. internship?

You match your internship and advanced PM&R program at the same time.

You match both in the same application and year. The MS 4 ranks PM&R “advanced” programs (to start in the PGY-2 year) on their primary ROL and for each advanced program also submits a supplemental ROL of prelim/transitional programs. If you match at a given advanced program then the NRMP computer algorithm will run through your supplemental list for that program. If all goes well on Match Day you will find out where you’ll be going for internship as well as for PM&R the following year.

Advanced PM&R residencies generally do count the traditional rotating internship. Not sure about the other scenarios - I'll defer to our DO colleagues in that regard.

most places will accept a DO internship year. so you can participate in the DO match for internship. Then do MD match for advanced residency spot. hope this helps.

A quick run down on residency application definitions:

Intern year: 1 year after med school that everyone must do. Usually consists of general inpatient experience. Teaches you to be a "real doctor". Lots of supervision by faculty and residents. Tends to be in Medicine, Surgery, and sometimes pediatrics
a. Preliminary: for those who need an intern year but will go onto a more specialized residency (pm&r, gas, rads, etc). Usually medicine or surgery.
b. Categorical: For those who will continue in the field. Medicine, Surgery, and pediatrics
c. Transitional: a smattering of rotations ranging from Internal medicine, to peds, to ob gyn. Usually with a few electives mixed in. Traditionally is an easier intern year. You know a lot about nothing. Filled up usually with Rads, and optho, and other non-inpatient specialties usually.

2. Residency: the other 2-5 years. What you will grow up to be.
a. Categorical: Same as in intern
b. Advanced: You did either transitional or prelim.
3. Fellowship: For those wishing to further specialize/do research.

As a MS IV, you are responsible for applying to both internship and residency. They are matched seperately, except for programs that have them combined (Some Pm&R, most medicine peds and surgeries).
Match day will tell you on line 1: You matched at _____ for your advanced residency. Line 2. You matched at _____for your internship.

Also, is matching into an advanced PM&R slot contingent upon matching into an internship? For example, would any applicant be turned away from the advanced PM&R slot if she failed to match to an internship, or just didn't attempt to match to one?

wrong,as i said, can apply for do internship-AOA match, and adv PMR-ACGME match in same year. if you dont match for internship then would have to scramble.or could just not apply at all then scramble. plenty go unfilled. matching for adv spot is not contingent on matching for internship. Another option is to save yourself the hassle and apply to only linked PMR through ACGME (internship included) hope this helps.
