I'm interested in PM&R and was wondering if working in a physiatrist's office would carry as much weight as gaining exposure to PM&R through a summer externship when it comes time to apply for a residency position.

A summer externship would probably give you a better overall exposure to PM&R, provide you with potential research opportunities, and help you form connections. That being said, don't give up your summer because you think you can't be a competitive applicant otherwise.
