I like the Therapeutics Education Collaboration. Most of the episodes are free, and then every fourth one is a "premium" podcast that you have to subscribe to get. I went ahead and sprang for the subscription, just because I like the podcast and wanted to support it, but I don't think it's absolutely essential to do that.
