Is it possible to be an academic surgeon and only perform and publish clinical research (in addition to your operative duties of course)? I'm not too excited about basic science research relating to surgery, but am very interested in the pure clinical research.

Will this kind of research keep you at the Assistant Professor level without advancement to the Associate Prof or full Prof level?

Must you do basic science research in order to advance up the tenure-track?

It's probably better to ask the attendings at your university. They will be the ones in the best position to give you accurate and current information.
 
The vast majority of research done by attendings at my institution is clinical. Some of it is with patients, while most of the rest is in the pig lab. And the day to day grunt work of the pig lab stuff is done by the research residents and fellows, not the attendings...they serve more as advisiors.
