There are some four-year programs that require four years of residency, and one four-year program actually requires four years of residency plus fellowship before you are considered for a position.

Not doing a fourth year might put you at a disadvantage for applying to some jobs (a minority), but doing an extra year will never put you at a disadvantage. Nobody is going to say "I'm not going to hire a four-year grad" whereas the opposite might not be true. ED directors and department chairs who graduated from four-year programs tend to like their own.

(This isn't a post about whether four-year programs are better than three-year programs. This is a post about reality, whether it's justified or not.)

The majority of ER physicians in academics were trained at 3 year programs. Need I say more?

To add to this comment. Having interviewed at some 15 programs this year, I was very impressed and surprised with the number of residents who were already accepted in various fellowships or who had already taken jobs at on faculty in other EM programs. You can absolutely go any route you want out of virtually any 3 year residency. You just have to pursue it and make it happen.

I think it comes down to "comfort" level. Some people just prefer to have the extra year to gain confidence and there is nothing wrong with that. People bring different levels of confidence and motivation to medicine. Some who come to medicine are already quite "fluent" in it and don't want to spend 4 years learning their trade. Quite frankly they don't need to. Others are young and have not had other life experiences to build their confidence, or just want the notoriety that comes with many of the big name 4 year programs. You can go into anything you wish out of 3 or 4 year programs and ultimately its not going to matter one bit in the long run. Everyone always talks about wasting a year of salary doing a 4 year EM program. Who really cares if its what you want? Its no big deal...as most of us don't enter medicine at the exact same age anyway. There are the old farts like FoughtFyr and myself, and then you have the superyoungins as well. Take it from someone who spent 8 years doing something else that an extra year of salary is a non-issue in the long run. Sure, some economics guru is going to debate what you could do with an extra 200K in the bank over 30 years, but none of us will ever be starving and all our kids are going to be at nice colleges. You can't take it with you anyway in the end!!

I can't speak for all programs, but nearly half of our residents (including myself) have had significant medical experience prior to entering medical school and joining our residency program.

r54918 - your rank list is basically made up of all of my favorite programs!! Any way I could persuade you to post or PM me with your stats (grades, boards, MD/DO, research)? Pretty please :-D. You would be helping out an *already* nervous 2nd year :-D

Q

Here are programs I am ranking. Although I did a decent number of interviews, I am only ranking 6-7.

Pitt
Yale
Brown
Sinai (NY)
NYU
BIDMC
MGH/Brigham

I though all the programs were great, but I thought Yale and Pitt were awesome and would be happy with either one as my first.

Here is where I interviewed.. In no particular order..

Resurrection
Cook
Wash U
Orlando
Harbor
USC
Arizona
Henry Ford..

I turned down a lot of interviews.. 😀

I think you turned down more interviews than I got offered....funny.

Well when you apply to over 100... it is a sure thing.. Esp when you consider I ONLY interviewed at these few..

I bet I turned down more than 90% of people applied to and I bet I got rejected from more than 50% of people applied to..

You are ranking Jacobi way too high. Don't do it. Zero (read Z E R O) support from administration. Residents are considered little more than cheap labor.

I think I'm adequately annonymous. Plus, this list is still changing slightly every couple of days. I can't stop messing with it. Anyway, here it is (for now).

1 Northwestern
2 Jacobi
3 Brigham/MGH
4 Boston Medical Center
5 Yale
6 Cook County
7 Mt Sinai
8 Penn
9 St. Luke's/Roosevelt
10 Downstate/Kings County

By the way, I hated how a lot of people said things like, "people who like northwestern won't like cook," or "people who like BMC won't like Brigham/MGH." I liked all of those programs a lot, for different reasons. I like that it's up to the "ranking gods" which program I will end up at. It makes my "decision" a lot easier.

For you non-4th years out there (specifically 3rd years). Know that you should read these rank lists not as a ranking of quality of programs, but as a list of individual impressions and personal factors. Also know that a lot of people will not rank certain programs. For me, that means I would be extremely happy ending up at any of the programs on my rank list. Any programs I didn't like, I didn't rank - simple as that.

Thanks for the opinion, Jaydoc. I know a good bit about Jacobi, and it will likely stay where it is on my list. I really enjoyed my time in both the Jacobi and Monte ERs - much more than my other rotations at "similar" hospitals. There was a great amount of teaching, and the residents (which I assume you are one of) were amazing.

If you are a current resident, I'm sorry you feel like there is little support on the part of the administration and that you feel overworked. I would love to hear more - please PM me. Just out of curiosity, are you from a non-NYC med school?

I say we start another thread for the ROL because this one is already making us all have to scroll too much. I'll get to it...pleaase wait to reply until tomorrow at noon...I'll keep it bumped up so its close to the top PRN.
