Is it okay to tell my top program that I am going to rank them #1. Is this considered a violation? I will tell my second and third program that I will rank them very high. I plan on sending an interest email in January.

Thank you.

This is totally fine. The only way this could be a match violation is if you say "I'll rank you #1 if you promise to rank me #1". Otherwise, you are welcome to tell programs anything you want. Programs can likewise tell you where you are ranked on their list.

I thought PDs can't tell you where you are ranked?

Oy vey. Here we go again. The rule is quite simple. You (and PDs) can say whatever you want to say. What they can't do is say "we will rank you to match if you rank us number 1" but they can say "we will rank you to match." There can't be coercion.

Honestly doubt it would help anyone very much, and in fact I think it can only hurt. Good chance people will think you are bs-ing them or saying this to multiple programs. I would just tell them something to let them know how interested you are like -- I would really be thrilled to match at your program, and I will be ranking you very highly. Helps if you provide reasons too.

I wouldn't say "I'll rank you highly". This implies that program is NOT #1 and may be insulting.

I only told my #1 that they were #1. I thankfully matched there. Who knows if it helped.

Honestly, I doubt anything you do will change their opinion of you much post-interview.

If you have told a program you intend to rank them 1, and then a life issue comes up that makes you reevaluate where your will rank 1 (ie marriage, pregnancy, family emergency etc), does this need to be addressed with the initial program? Is there a standard way to address it?

You could just follow the lead of programs who make certain statements/promises and do whatever you want even if it is the opposite of what you said.

You could let them know your situation, but I would guess telling them that they have dropped on your rank list is a great way to get dropped on theirs.
