I'm interested in dermatology, but I have little interest in the cosmetic side of things. Is it common or reasonable to practice dermatology without performing any cosmetic procedures? And if one does so, would you expect there to be a drop in income compared to your average dermatologist?

You will have no trouble practicing derm with zero cosmetics if you want to do that (I was same - had plenty of exposure but zero interest). You might experience a small drop in income particularly in private practice - less so in a large system or academics.

It shouldnt be that significant though - don’t forget most dermatologists do about 5-10% cosmetics and the rest is medical.

It's super common. Don't even worry about it.

Income difference is quite small when compared to a general derm that does a little cosmetics.
