Prior to applying to a medical school, would it be wise or unwise to email the admissions officer on your desire to attend their medical school? Or does this come off wrong?

It will absolutely do zero to help you. It most likely won't do anything to hurt you either, unless on the very very off chance the make a note of it to include in your file when you do apply, or happen to remember the strange, pointless letter/email from someone who hadn't even applied yet.
The only useful thing you could do is try to get a meeting with someone in admissions to review your potential as an applicant before you apply. If they actually do meet with pre-meds to advise you, they could give you pointers on how to apply with the best application possible. If they won't meet with you other than that, don't bother sending a "intend to attend" note to them as it won't help, and might hurt, even if that is a slim chance. Really, there is nothing to gain so it is pointless. Your application is the only thing they really care about, not a pre-application letter.
