2 semesters anatomy/physiology
2 semesters chemistry
2 semesters physics
1 or 2 semesters biology
1 semester psychology
1 semester statistics

Those are the most common pre-reqs. It varies between schools.

The vast majority of PT programs require the GRE (a few don't). 150V 150Q 3.5AW is generally a qualifying score but more competitive programs require a higher score. The test has a math section, a language section, and a writing section.
