Honestly, it depends on the school, and you'll have to contact each individually to see what they require. As people have stated in this thread, UCSF doesn't have a policy regarding this. For the other schools I either applied or was considering, UOP requires retaking of organic chem and 1st year bio if it's been more than 7 years (though for bio you can substitute anatomy or physiology, or retake 1st year bio). UCSD has a strict 10 year policy for all science and math classes, which is why I didn't bother applying. If I remember correctly, Hawaii-Hilo and Touro-California didn't have a policy either, and Creighton had a 5-7 year policy.
It was 12-15 years since I had first taken these classes, and I just retook all the classes I got C's and D's in myself. Luckily I got A's and B's which helped me get the interview for UCSF. 🙂
