In what cases are either of these contraindicated and so on? I feel like a prefab could be used in just about all cases.
 
From my understanding, if the shape of the canal space makes it difficult to fit a prefab post in without removing excessive dentin, such as with a ribbon or C-shaped canal. The other times would be if there is a concern with rotation. Cast post-cores have better anti-rotation properties.
 
I see...I also learned that cast post and core are more likely to fracture the root. I guess each has its ups and downs.

Tapered root form posts are more likely to fracture the root than parallel sided posts, which is why case selection is important; if you can't maintain good ferrule, than maybe it's not a good idea to shove a tapered spike into the root.

The choice of pre-fabricated or custom depends on this very important factor. That is how stable the post is in the prepared cannal. If you look at the prefab kit, each post has its own drill (color coded). This is to ensure the prefab post fits almost tightly to the prepared canal. If the prefab post is too small for the canal or has too much "play", then you need to have a custom one. Regardless of the choice, you must have ferrule to prevent the crown from being fractured off. This requirement is much more important than the choice of the post itself. DP
 
I think everyone uses only pre-fabs in the real world. Cast P&C is a dental school thing for the most part.
 
Pre-fab pt's come in with posts in their hands. Cast patients come in with a split tooth. Which would you prefer Doc?
 
Servitup prefers that all patients get cast post and cores, so he can pull those sukkas👍
