prelim or transitional for psychiatry???

When you apply for advanced programs in psychiatry where and how do you get you PGY-1 then? do you apply for prelm or transitional? can you do that for psychiatry? or you start psych residency at other program and then apply for advanced?
 
Psychiatry is a 4-year categorical residency which starts with PGY-1.

The PGY-2 positions I know of are filled by people switching into psych from other specialties.
 
Hey thanks for your reply! the thing is that I have a fair chance to get into a top psych program but only as a PGY 2 what do I do for PGY 1 then? any thoughts? I'm lost
 
That doesn't make much sense to me. Are you a US senior? FMG/IMG? Is this a specific program that you are being offered a spot at? Or is this some sort of general hypothetical?
 
yeah I am sure it doesn't. FMG , yes it is a specific program that says I have better chances to get in as PGY 2 and none as PGY 1 but what do I do for PGY 1 then? transitional? prelm? transfer?
 
If you are certain that psych is what you really want to do, I would apply broadly to categorical PGY-1 psych positions through ERAS and the Match, and try to get a position that way. Since you are a FMG, if a program really likes you, they could even sign you for a PGY-1 position outside the Match, if they really wanted to.

If you are not a very competitive applicant and are concerned about not matching into a PGY-1 psych spot at all, then I would simultaneously apply to prelim medicine spots as a backup, with the idea of transferring into a psych program later as a PGY-2.

If you really think you are not competitive enough to match into a PGY-1 psych spot, I wouldn't waste your time/money applying to transitional year programs, as they tend to be much more competitive.

Why do I advise that you really try and match into psychiatry as a PGY-1? Because our PGY-1 year includes 6 months of psychiatry (plus 4 months internal medicine and 2 months neurology). Those who enter psych at the PGY-2 level from other specialties start 6 months behind on their psych requirements. This generally translates into much less elective time for them so they can get the requirements done.

If psych is your passion, I think you are likely to get a much fuller educational experience by doing all 4 years of psych residency, IMHO. Just my 2 cents...take it for what it's worth.

Personally, my "BS" alert is going off here. Why would some top program be willing to take you as a PGY-2 rather than a PGY-1? That doesn't make much sense, and it may simply be their way of telling you no.

So, I agree with the above advice. Find a PGY-1 at a program you'd be happy completing. If this works out as a transfer as a PGY-2, great, but I wouldn't count on it.
