Will doing a preliminary surgical internship at an institution make it easier to get a spot in their anesthesiology program granted the person shows interest from the very beginning and talks to the PD early on about wanting to do anesthesiology after the internship. the only negative would be that they may have to repeat the internship, right? any ideas?

I don't know if you can repeat your internship. You may have to apply for an advanced position while doing your prelim in surgery, I know some prelim surgery programs that have 2 years, Not quite sure about this.. 🙄
