Premarin Vag BID?

Hospital pharmacist. Have already seen twice Premarin vaginal, 1 gram vaginally twice daily for a month. Can't find any reason to do this, first MD realized it and changed it, second one said that's what they wanted, but with no explanation. I'm curious, any ideas? Thanks
