Preop EKG requirements

As far as your policy what guidelines are you utilizing for elective surgery? Do you require them for all patients over 40 years of age? Do you require one within two months for a patient with a cardiac history? What about a healthy patient who has had an EKG within the last 1-2 years? I have been reviewing Sweitzers handbook on preoperative assessment as far as EKG studies are concerned.

This is what we use at our university's anesthesia preop clinic (incidentally, directed by the aforementioned Dr. Sweitzer):

I would say, though, that we're much more liberal than the guidelines and order more EKGs than would be recommended by them.
