I recently presented a poster. My PI wants me to present it at another conference completely unchanged. Would I list the poster name twice with a different location? And do adcoms care about this at all, or does it look no better than if I just presented it once?

You would list the poster only once under the more prestigious location of the two and then mention at the end of the same space that you also presented it at XXXX Conference in Fresno 4/12/201x.

So is it a better idea to slightly change the poster and present it under a different title, or is that completely transparent?

Since it's the same data, it wouldn't matter if the title was different. You don't want to be perceived as having fluffed up your application.

On the other hand, someone working on an extensive project over the period of many years may have very similar sounding poster titles but not be presenting the same work over and over. It's tricky to judge someone's research from their resume

It would be best to simply state in the description "I have presented this poster twice, the first time .... and the second time .... "

That way you aren't doing anything dishonest.

If the dates were close to each other, most people would find it hard to imagine that the project substantially improved over that time.

Giving one presentation is fine, there's no need to fluff something like this. You could take Spinach Dip's suggestion and simply address that the presentation was given twice, which isn't bad. It's not something worth giving doubt for whoever is evaluating your CV that you're filling it with fluff.

Perfectly legit thing to do. I can do it for my school's own annual research forum, for the AOA Convention, and for my own professional society's annual meeting.
