yes, the amcas automatically uploads your MCAT scores to file. you can send primaries before you receive your score and you probably should (at least at some schools)
Yes and i'm pretty sure you can add more schools after you get your MCAT (even though your AMCAS is submitted). That way if you do better or worse than expected you can add schools you are more competitive at.

Do you ever get secondaries before your score is out? Do you mean by "at some schools" those schools that don't screen and send secondaries to everyone, so your MCAT score wouldn't be needed?
At the very least, submit early still so your AMCAS isn't held up after your scores are back, when everyone is sending their AMCAS at the same time in July/August. I do believe you can add/delete schools as necessary after receiving your scores....or at least decide not to send in secondaries if you ge them.
